Job summary

Region VII Area Agency on Aging/Sunrise PACE is accepting applications for a motivated Intake Coordinator to evaluate and coordinate new enrollees, provide education to enrollees and families, and assist with the enrollment process.

The Sunrise PACE center and job location are in Fort Gratiot, MI, and the role requires teamwork, excellent communication, and guidance on Medicare/Medicaid; prior experience with frail or elderly populations is preferred.

Qualifications

• Must possess excellent communication, interpersonal and conflict resolution skills.
• Knowledgeable of Medicare and Medicaid preferred.
• Ability to stay organized within busy environment.
• Establish and maintain effective relationships with staff, participants and the public.
• Clear written and verbal communication across diverse cultural backgrounds.

Responsibilities

• Evaluates potential enrollees to determine needs and eligibility for enrollment.
• Conducts assessments in the homes of potential enrollees.
• Coordinates all level of care decisions with State SHS personnel.
• Participates in interdisciplinary team and related meetings.
• Participates in marketing activities to recruit new participants.
• Completes documentation related to participant assessments.
