Innovative Integrated Health, Inc. is seeking an Intake Specialist to manage enrollment processes for senior participants in our PACE program. You will handle intake forms, eligibility verification, and coordination with the interdisciplinary team to ensure smooth enrollment and ongoing care planning.

The role requires proficiency in MS Office, strong communication, and meticulous record-keeping. You will collaborate with the marketing and care teams to meet enrollment targets while complying

Qualifications

• Minimum of one year of experience working with a frail or elderly population.
• High School Diploma or GED required.

Responsibilities

• Answer calls from prospective enrollees and explain eligibility requirements and services.
• Schedule intake meetings with prospective enrollees and conduct site tours.
• Complete intake assessments and verify Medi-Cal and Medicare coverage.
• Update PrimeSuite and PaceLogic with enrollment demographic data.
• Schedule Level of Care assessments and other pre-initial assessments as required.
• Complete enrollment conference with prospective enrollees.
• Update intake tracking logs for enrollment reporting.
• Support meeting enrollment targets and business development goals.
• Assist with participant recruitment activities and outreach.
