The role involves accepting referrals, completing Medicaid-related onboarding, coordinating participant visits, and collaborating with DSS, hospitals, and social services to ensure compliant long-term care eligibility.

Qualifications

• Associates Degree preferred or equivalent experience, training and/or education.
• Minimum of 1 year experience working with the elderly.
• Knowledge of Medicare and Medicaid regulations and screening process for long-term care.
• Must have valid driver's license and reliable transportation.
• Requirements include home visiting, analytical thinking, problem solving, and teamwork.
• Must be medically cleared for communicable diseases and up-to-date immunizations.
• Must undergo a criminal background check.

Responsibilities

• Accept incoming referrals from participants, family and/or other sources.
• Complete intake and follow-up documentation to verify PACE eligibility.
• Ensure new participants meet Long-Term-Care requirements with DSS, including Medicaid application and gathering required documentation.
• Coordinate visits by potential participants and caregivers to the PACE center.
• Maintain working relationships with workers at other health and social service agencies.
• Conduct PACE presentations in the community with DSS, hospitals, physicians and families.
• Participate in staff meetings and in-service training.
• Assist social workers with various tasks as needed.
• Make referrals to community and agency resources.
• Collect data on referrals and potential enrollees who did not enroll.
• Maintain confidentiality concerning all information pertaining to participants.
• Weekly reporting on client's progress to Directors.
• Assist in procuring ramps for PACE program.
• Coordinate and place PERS orders for All-CARE for Seniors and Mount Rogers PACE, maintaining a log of orders.
• Demonstrate good work ethics.
