Job summary

Region VII Area Agency on Aging, Sunrise PACE is seeking an Intake Coordinator to evaluate and coordinate new enrollees for the Sunrise PACE program at the Fort Gratiot, MI location. You will work with the intake partner to educate enrollees and families and assist with enrollment.

Responsibilities include in-home assessments, coordinating care decisions, and supporting marketing efforts to recruit participants while maintaining accurate documentation. Equal opportunity employer.

Qualifications

• Must work well in a team environment.
• Excellent communication and interpersonal skills.
• Knowledge of Medicare/Medicaid a plus.
• Ability to stay organized in a busy work environment.
• Ability to interact with diverse staff, providers, and the public.

Responsibilities

• Evaluate enrollees to determine needs and eligibility for enrollment.
• Conduct home assessments of potential enrollees.
• Coordinate care decisions with State Health and Human Services.
• Participate in interdisciplinary and management team meetings.
• Assist with marketing to recruit potential participants.
• Complete required documentation related to assessments.
