Job summary

The PACE Intake Coordinator at Lafayette Franciscan Healthy Living Clinic plays a key role in initial contact with referrals, guiding enrollment, and helping families access Medicaid services. The position collaborates closely with the PACE Center Manager and interdisciplinary team to advance enrollment and ensure timely completion of assessments and documentation.

Travel up to 50% may be required as part of outreach to communities served by the PACE program, with emphasis on accurate data

Qualifications

• High School Diploma or GED required.
• Associate’s Degree preferred.
• Minimum 1 year working with frail or elderly required.
• 2 years Medicaid (Nursing Home/Waiver) preferred.
• Driver’s license required.

Responsibilities

• Receives and processes referrals; follows up with potential participants regarding enrollment.
• Manages the work queue in EPIC for referrals and tracks enrollment status.
• Verifies eligibility of program applicants per state and federal regulations.
• Completes Home Intake Assessments and Level of Care assessments for potential participants.
• Reports census data to the State Administering Agency as required.
