Job summary

Element Care is seeking an Enrollment Coordinator to join the team and drive the PACE enrollment process. The role involves explaining the PACE model to prospective enrollees and caregivers, verifying eligibility, and coordinating with the enrollment team to ensure timely completion of enrollment steps.

The position requires at least one year of experience with seniors, strong communication skills, and proficiency with Microsoft Office and CRM software.

Qualifications

• Associates degree required; Bachelor’s in social work, sociology, psychology, business, public health, or related field preferred.
• Minimum one year of experience working with seniors and caregivers in healthcare/mental health settings.
• Proficient with Microsoft Office and CRM systems; able to learn agency-specific software.

Responsibilities

• Explain the PACE enrollment model to prospective enrollees and caregivers.
• Ensure eligibility criteria are met per enrollment regulations.
• Coordinate with enrollment team and IDT to monitor progress of referrals.
