Job summary

Lincoln Trail Behavioral Health System is a 140-bed hospital in Kentucky providing inpatient care for adolescents, adults, military, veterans and first responders. The Intake Coordinator oversees service requests, assesses patient needs, and liaises with managed care organizations and other payers.

The role includes assisting in admissions, documenting care plans, ensuring patient safety, and coordinating with RNs and physicians.

Qualifications

• Graduate Degree in social work, mental health/behavioral sciences, or related field.
• Licensed to practice as an RN in Kentucky.
• Ability to read, analyze and interpret scientific and clinical information.
• Ability to communicate effectively with physicians, patients, referral sources, and staff from various disciplines.
• CPR certification and completion of HWC within 90 days of employment.

Responsibilities

• Participates in patient admission procedure, vital signs, collects lab specimens, assists with ADLs, and prepares patients for exams.
• Ensures patient safety by identifying and reporting potential for self-injury to an RN.
• Demonstrates basic knowledge of emotional and mental health concepts.
• Completes all required documentation clearly reflecting patient goals and treatment plans.
