Lincoln Trail Behavioral Health System in Radcliff, KY, is seeking an Intake Coordinator to facilitate admissions, assess patient and family needs, and coordinate with managed care organizations. This role involves clinical liaison work to ensure appropriate services and documentation at intake.

You will work with psychiatrists, licensed therapists, nurses, and support staff to ensure timely access to services and high-quality care for adolescents and adults.

Formación

• Graduate degree in social work or related field required.
• Active Kentucky RN license required.
• Strong ability to read, analyze and interpret clinical information.
• CPR certification and HWC within 90 days of employment.

Responsabilidades

• Participates in patient admission, vital signs, lab specimens, and preps patients for exams.
• Ensures patient safety by reporting potential for self-injury to RN.
• Demonstrates knowledge of mental health concepts and treatment plans.
• Completes documentation clearly reflecting patient goals and treatment plans.
