Job summary

Lincoln Trail Behavioral Health System is seeking an Intake Coordinator to facilitate service requests, assess clinical needs, and interface with managed care organizations. The role collaborates with RNs and physicians to ensure smooth admissions and accurate documentation.

The position emphasizes care coordination, patient/family support, and adherence to treatment plans within a hospital setting in Kentucky.

Qualifications

• Graduate degree in social work or related field and/or licensed RN in Kentucky
• Ability to read, analyze and interpret scientific information and patient reports
• Ability to respond to inquiries from customers and regulatory bodies
• Ability to communicate with physicians, patients and staff across disciplines

Responsibilities

• Facilitate requests for services and assess patient/family clinical needs
• Interface with managed care organizations and payers
• Participate in patient admission, vital signs, and initial prep activities
• Document clearly and align with patient goals and treatment plans
