Job summary

Lincoln Trail Behavioral Health System, a 140-bed hospital, seeks an Intake Coordinator to facilitate service requests, assess needs, and interface with managed care payers. The role involves admissions procedures, safety oversight, and documenting patient goals within the treatment plan.

Qualifications

• Graduate degree in social work, mental health/behavioral sciences, or related field.
• Currently licensed to practice as an RN in Kentucky.
• Ability to read, analyze, and interpret common scientific and technical journals, patient evaluations and information reports.
• Ability to respond to inquiries from customers, regulatory agencies, or community members.
• Ability to communicate effectively with physicians, patients, referral sources, and staff from various disciplines.
• CPR certification completed and HWC within 90 days of employment.

Responsibilities

• Participates in patient admission procedures, vital signs, lab specimen collection, assists in ADLs, and prepares patients for diagnostics.
• Ensures patient safety by reporting potential self-harm to an RN.
• Demonstrates basic knowledge of emotional and mental health concepts.
• Completes all required documentation clearly and concisely reflecting patient goals and treatment plans.
