Job summary

Ascend Health in Worcester, MA is seeking an Intake Coordinator to manage initial inquiries, convert referrals to admissions, and support patients and families through the admissions process. You will collaborate with hospice staff to obtain consents and ensure smooth intake workflows.

The ideal candidate has an associates degree, 5–8 years of related experience, a valid driver’s license, and strong communication, interpersonal, and organizational skills.

Qualifications

• Associates Degree in a related field or equivalent experience.
• 5–8 years of experience in a related capacity.
• Valid driver's license and reliable transportation.

Responsibilities

• Completes initial inquiry forms for all potential admissions, and forwards completed forms to the appropriate Hospice Nurse and Director of Clinical Services and/or Administrator.
• Insures that whenever possible, all referrals are converted to admissions
• If referrals do not convert to admissions, the Administrator will be notified
• Track all inquiries and referrals daily. This report will be given to the Administrator weekly
• Assists patients, families and responsible parties in completing the admissions packet as needed
• Works collaboratively with appropriate staff to insure written or verbal consents are obtained
