Job summary

Ascend Health is hiring an Intake Coordinator to manage the front end of our home health admissions. You will complete initial inquiry forms, forward them to clinical staff and administration, and work to convert referrals into admissions.

This role requires travel, a valid driver's license, strong communication, interpersonal and organizational skills, and experience in intake or admissions in healthcare. We offer competitive pay, benefits, and opportunities to grow with our team.

Qualifications

• Associate degree preferred.
• Five to eight years of related experience preferred.
• Experience in intake, admissions, healthcare, or related capacity preferred.
• Valid driver's license and reliable transportation required.
• Must be willing to travel.
• Excellent communication, interpersonal, and organizational skills.
