Job summary

Heart to Heart Hospice in Plano, TX is seeking an Intake Coordinator to oversee daily patient referrals and intake processes, help implement intake policies, and ensure accurate charting in the EMR. The role requires 3–5 years of healthcare experience, a health-care related degree preferred, and strong knowledge of CHAP standards and payer practices.

Qualifications

• Degree in health care or related field preferred.
• 3–5 years of experience in health care, preferably in hospice operations.
• Admissions/Intake and customer service experience.
• Valid Driver’s License and Automobile Insurance.
• Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices.
• Experience with EMR software.

Responsibilities

• Direct all daily patient referral and intake operations including participating in the establishment and implementation of intake policies.
• Initiate chart into EMR and maintain workflow.
• Maintain census, pending admissions, red flag, journey’s end and care plan meetings on stand up board.
• Ensure compliance with state, federal and CHAP as applicable referral/intake regulatory requirements.
• Establish and maintain positive working relationships with referral sources and monitor community and customer to enhance perceptions of Heart to Heart Hospice as a high-quality provider.
• Gather, collate, and report referral statistics including key customer referral trends.
• Participate in strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs.
• Maintain comprehensive working knowledge of company contractual relationships and ensure patients are admitted according to contract provisions.
• Participate in quality improvement teams and activities.
• Maintain knowledge of community resources and assist referral sources in accessing them when services are not provided by Heart to Heart Hospice.
• Timely submission of all documentation.
• Performance of other duties as required.
