Job summary

Ascend Health in Glen Allen, VA is seeking an Intake Coordinator to manage inquiries, forward forms to the Hospice Nurse and Director, and help convert referrals to admissions. The role requires coordinating with staff to secure consents and assist families through the admissions process.

Responsibilities include daily tracking of inquiries and referrals, assisting patients with admissions packets, and ensuring timely communication with administrators.

Qualifications

• Excellent communication, interpersonal and organizational skills.
• Experience in a related capacity is preferred.
• Must have a Valid Driver's License and reliable transportation.
• Must be willing to travel.

• Completes initial inquiry forms for all potential admissions and forwards forms to the Hospice Nurse and Director.
• Ensures referrals are converted to admissions whenever possible.
• Tracks inquiries and referrals daily; reports to the Administrator weekly.
• Assists patients and families with completing admissions packets.
• Collaborates with staff to obtain written or verbal consents.

Intake Coordinator

• Completes initial inquiry forms for all potential admissions, and forwards completed forms to the appropriate Hospice Nurse and Director of Clinical Services and/or Administrator
• Insures that whenever possible, all referrals are converted to admissions
• If referrals do not convert to admissions, the Administrator will be notified
• Track all inquiries and referrals daily. This report will be given to the Administrator weekly
• Assists patients, families and responsible parties in completing the admissions packet as needed
• Works collaboratively with appropriate staff to insure written or verbal consents are obtained

Education:

Associates Degree in a related field

Professional Experience:

• Preferably 5-8 years
• Experience in a related capacity
