Job summary

Affinity Health Management is seeking an experienced Intake Supervisor to lead the patient intake process and manage the intake team in a hospice care setting.

You will coordinate with referral sources, marketing, and clinical teams to ensure smooth patient transitions, while ensuring compliance with CHAP and payer requirements. The role requires strong leadership and communication skills and a proactive approach to resource access.

Qualifications

• Minimum of 3 years’ experience in health care, preferably in home care operations.
• Demonstrated ability to supervise and direct professional and administrative personnel.
• Ability to market aggressively and deal tactfully with customers and the community.
• Knowledge of CHAP standards, governmental regulations and private payer practices.
• Demonstrates strong communications, negotiation and public relations skills.
• Shows autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.

Responsibilities

• Directs all daily patient referral and intake operations including oversight of intake policies implementation.
• Enters referral and patient information into electronic medical record systems and related applications.
• Coordinates with referral sources, marketing and clinical teams to ensure patient transition into hospice care.
• Ensures compliance with state, federal, and CHAP referral/intake regulatory requirements.
• Oversees implementation of improved work methods to admit patients per policy.
• Maintains positive relationships with current and potential referral sources.
• Oversees insurance verification and authorization processes.
• Keeps up-to-date with Continuum Care’s contracts and ensures patient admission per contract provisions.
• Assists referral sources in accessing community resources when services are not provided by Continuum Care.

Responsible for managing all aspects of the patient intake process including managing the members of the intake team, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.

Essential Job Functions/Responsibilities

• Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
• Enters referral/patient information into electronic medical record system and other applications as needed.
• Coordinates with referral sources, marketing and clinical teams to ensure patient transition into hospice care.
• Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements.
• Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
• Establishes and maintains positive working relationships with current and potential referral sources.
• Ensures direct oversight of insurance verification and authorization processes.
• Maintains comprehensive working knowledge of Continuum Care’s contractual relationships and ensures that patients are admitted according to contract provisions.
• Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Continuum Care.

Position Qualifications
