Ascend Hospice & Palliative Care MA is seeking an Intake Coordinator in Worcester, MA to manage referrals and admissions. The role involves processing initial inquiry forms, coordinating with nursing and administration, and ensuring smooth conversions from referrals to admissions.

Responsibilities

• Complete initial inquiry forms for all potential admissions and forward to the Hospice Nurse and Director of Clinical Services/Administrator.
• Ensure referrals are converted to admissions where possible.
• Notify Administrator if referrals do not convert to admissions.
• Track inquiries and referrals daily; report to the Administrator weekly.
• Assist patients and families in completing admissions packets as needed.
• Collaborate with staff to obtain written or verbal consents.

Now Hiring - Intake Coordinator

TheIntake Coordinatoris responsible for, but not limited to:

1. Completes initial inquiry forms for all potential admissions, and forwards completed forms to the appropriate Hospice Nurse and Director of Clinical Services and/or Administrator
2. Insures that whenever possible, all referrals are converted to admissions
3. If referrals do not convert to admissions, the Administrator will be notified
4. Track all inquiries and referrals daily. This report will be given to the Administrator weekly
5. Assists patients, families and responsible parties in completing the admissions packet as needed
6. Works collaboratively with appropriate staff to insure written or verbal consents are obtained

Education:

Associates Degree in a related field

Professional Experience:
