Liberty Home Care is seeking an experienced Central Intake Scheduler to manage admissions and routine visits for hospice, home health, and palliative patients. The role emphasizes prioritizing urgent and same-day requests and maintaining clear communication with referral sources and families.

The position requires strong organizational skills, attention to detail, and experience with healthcare scheduling and EMR systems.

Formación

• High school diploma or equivalent required.
• Associate degree or healthcare-related coursework preferred.
• Experience with Microsoft 365, medical scheduling, admissions coordination, or hospice/home health preferred.

Responsabilidades

• Coordinate and schedule all admissions and routine visits for hospice, home health, and palliative patients, prioritizing urgent and same-day requests.
• Maintain proactive communication with referral sources, marketing team, field clinicians, intake staff, and patient families for clarity and timely follow-up.
• Input and update scheduling activities within the EMR to support data integrity and regulatory standards.
• Anticipate and resolve scheduling conflicts or delays to prevent service disruptions.
• Collaborate with interdisciplinary teams to support smooth transitions of care and staff assignments.
• Maintain organized documentation of all scheduled admissions and completed visits for tracking and reporting.
• Perform additional duties to support departmental goals and patient care outcomes.
