Job summary

CareRing Health, LLC is seeking an Intake Coordinator to collect and verify admissions data, coordinate with referral sources, and manage patient insurance information. Qualifications

• HS diploma or equivalent; an Associate's degree is preferred.
• Active RN or LPN license.
• 2+ years of experience in a similar intake role.
• Knowledge of Medicare, Medicaid, PPO, HMO and Advantage plans.

Responsibilities

• Collect and verify intake data for admissions per SOP.
• Obtain demographic, financial, clinical data via interviews with referral sources.
• Verify patient insurance through payer websites and calls.
• Perform clerical work and document management.
• Prepare and maintain patient charts in non-clinical functions.
• Track new patients and status of paperwork and benefits in EMR.
• Update EMR systems timely and accurately.
• Coordinate facilities, hospitals, and referrals to schedule providers.
• Maintain geographic scheduling for assigned practices.
• Answer phones to promote admission efficiency and report issues to supervisor.
• Log referrals and compile monthly statistics.
• Represent department in a professional manner and attend meetings.
• May perform other duties as needed.
