Job summary

Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/Registered Nurse (RN) to coordinate comprehensive care for inpatients and manage patient transitions. The role emphasizes collaboration with the care team to ensure appropriate service use and quality outcomes.

The position requires an RN license (VA/NC/Compact), BLS within 90 days, and a Case Management certification within one year. A Bachelor's degree in Nursing is preferred.

Qualifications

• 1 year of nursing experience is required.
• 1 year of acute care case management experience preferred.
• RN license in VA/NC or multi-state compact is required.
• BLS must be obtained within 90 days of hire.
• Certification in Case Management within one year of eligibility.

Responsibilities

• Coordinate and facilitate comprehensive care management for inpatients.
• Support patients transitioning to appropriate levels of care.
• Collaborate with healthcare team and patients to optimize LOS and service use.
• Develop and implement a comprehensive care plan addressing physical, mental, and psychosocial needs.
• Operate in hospitals-only setting as part of the inpatient care team.
