Qualifications

• Registered Nurse (RN) license required.
• Compact/multi-state license or Virginia state license required.
• Basic Life Support (BLS) certification required.
• Case Management certification within one year of eligibility.
• Minimum 1 year nursing experience required.

Responsibilities

• Coordinate comprehensive in-patient care management.
• Facilitate transitions to appropriate level of care.
• Collaborate with ED, nursing, case management, and health plan teams.
• Develop and implement care plans addressing physical, mental, and psychosocial needs.
