Job summary

Sentara Princess Anne Hospital in Virginia Beach, VA, is seeking an Inpatient Case Manager RN for 40 hours weekly, dayshift, with rotating weekends. A sign-on bonus of up to $20,000 is available for qualified candidates. Requires RN license in VA/NC or multi-state, BLS, and CM certification within a year.

Bachelor’s in Nursing preferred, 1 year nursing experience required. The role focuses on care management and patient transitions, collaborating with the care team to optimize LOS and outcomes

Qualifications

• Bachelor's degree in Nursing preferred.
• RN license in VA or NC or Compact/Multi-state required.
• BLS certification within 90 days of hire.
• Case Management (ACM/CCM/CCCTM/CMAC/CMGT-BC) within one year of eligibility.
• 1 year of nursing experience required; acute care or case management preferred.

Responsibilities

• Coordinate inpatient care and transitions across the continuum.
• Develop and implement comprehensive care plans addressing physical, mental, and psychosocial needs.
• Promote appropriate utilization and monitor health services and LOS.
• Collaborate with healthcare teams and patient/caregiver to manage transitions.
