Sentara Norfolk General Hospital is seeking an Inpatient Case Manager/Registered Nurse (RN) to coordinate comprehensive care management and patient transitions within the hospital setting. The role emphasizes collaboration with the care team to ensure clinical appropriateness and efficient LOS.

Qualified candidates will hold an RN license in VA/NC/compact, be BLS-certified within 90 days, and obtain a Case Management certification within one year of eligibility.

Qualifications

• Bachelor's Degree in Nursing preferred.
• Active RN license in VA, NC, or Compact/Multi-state REQUIRED.
• BLS within 90 days of hire.
• Case Management certification required within one year of eligibility.
• 1 year of nursing experience.
• 1 year acute care case management preferred.

Responsibilities

• Provide and facilitate comprehensive care management across the healthcare continuum, focusing on patient transition to appropriate level of care.
• Directly support in-hospital patients identified for comprehensive case management.
• Promote effective utilization and monitoring of health services through collaboration with the healthcare team and patient/caregiver.
• Develop and implement a comprehensive care plan addressing physical, mental, and psychosocial needs.
