Job summary

Sentara Norfolk General Hospital in Norfolk, VA is seeking an Inpatient Case Manager/Registered Nurse for a full-time weekend dayshift. The onsite role at 600 Gresham Drive offers 36 or 40 hours with a first-week training period and a weekend shift differential.

The nurse will coordinate care across the continuum, optimize transitions, and develop care plans addressing physical and psychosocial needs. RN licensure in VA/NC/Compact is required.

Qualifications

• 1 year of nursing experience.
• 1 year acute care case management preferred.

Responsibilities

• Provide comprehensive care management across the patient care continuum with focus on transitions.
• Directly support in-hospital patients identified as needing case management.
• Promote effective utilization and monitoring of health services; collaborate with the healthcare team and patient/caregiver to manage care and transitions.
• Develop and implement a comprehensive care plan addressing physical, mental, and psychosocial needs.
• Functions in Hospitals Only.
