Sentara Norfolk General Hospital is seeking an Inpatient Case Manager RN to coordinate comprehensive care management and patient transitions in an on-site role in Norfolk, VA. The position offers a $20,000 sign-on and a full-time schedule with rotating weekends, 8:00 AM–4:30 PM EST.

Requirements include a Bachelor’s in Nursing (preferred), RN licensure in VA/NC or multi-state, and BLS within 90 days of hire; ACM/CCM certification within one year.

Competenze

• Bachelor's degree in Nursing preferred.
• RN license in VA/NC or Compact/Multi-state required.
• BLS within 90 days of hire; ACM/CCM certification within 1 year of eligibility.
• 1 year of nursing experience; 1 year acute care case management preferred.

Mansioni

• Provide care management across the hospital stay with focus on patient transitions.
• Directly support inpatient patients identified for case management needs.
• Collaborate with healthcare team and patient/caregiver to manage care and transitions.
• Develop and implement a comprehensive care plan addressing physical, mental, and psychosocial needs.
