Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/Registered Nurse (RN) for the Full Time Weekend Dayshift with a potential $20,000 sign-on. The role focuses on coordinating comprehensive care, patient transitions, and collaboration with the care team across a 3x12 or 4x10 weekend schedule.

Requirements include RN licensure in VA/NC/Compact, BLS, and ACM/CCM/CCCTM/CMAC/CMGT-BC certification within a year.

Qualifications

• Bachelor's Degree in Nursing preferred.
• Registered Nurse (RN) in VA or NC or Compact/Multi-state REQUIRED
• BLS within 90 days of hire.
• Case Management certifications ACM/CCM/CCCTM/CMAC/CMGT-BC required within 1 year.
• 1 year of nursing experience REQUIRED
• 1 year acute care case management preferred

Responsibilities

• Provide and facilitate comprehensive care management for patients across the healthcare continuum.
• Focus on patient transitions to appropriate level of care and LOS management.
• Collaborate with the healthcare team and patient/c caregiver to manage care and transitions.
• Develop and implement a comprehensive care plan addressing physical, mental, and psychosocial needs.
