Qualifications

Responsibilities

• Manage and facilitate comprehensive care management and transitions.
• Support in-hospital patients requiring case management.
• Promote appropriate utilization of health services and communicate with the care team.
• Develop and implement a care plan addressing physical, mental, and social needs.
• Operate in Hospital setting with focus on inpatient care.
