Job summary

Sentara Norfolk General Hospital is seeking an Inpatient Case Manager RN for a part-time weekend dayshift. The role focuses on care coordination, discharge planning, and optimizing patient transitions within a large tertiary center.

Eligible for a sign-on bonus and weekend differential, with a strong emphasis on teamwork and patient safety. The position requires RN licensure in VA/NC or a multi-state compact, a BSN preferred, and ACM/CCM/CCCTM/CMAC/CMGT-BC certification within one year.

Qualifications

• Bachelor’s Degree in Nursing preferred.
• RN license in VA or NC or a multi-state compact.
• CCM/ACM/CCCTM/CMAC/CMGT-BC certification required within 12 months of eligibility.
• 1 year of nursing experience; 1 year in acute care case management preferred.

Responsibilities

• Coordinate and facilitate comprehensive care management for inpatients.
• Support patient transitions to appropriate level of care and ensure clinical appropriateness.
• Collaborate with healthcare team and patient/caregiver to manage care and transitions.
• Develop and implement a comprehensive care plan addressing physical, mental, and psychosocial needs.
