The position requires an RN license (VA/NC/Compact), BLS within 90 days, and a Case Management certification within one year. A Bachelor's degree in Nursing is preferred.

Qualifications

• 1 year of nursing experience is required.
• 1 year of acute care case management experience preferred.
• RN license in VA/NC or multi-state compact is required.
• BLS must be obtained within 90 days of hire.
• Certification in Case Management within one year of eligibility.

Responsibilities

• Coordinate and facilitate comprehensive care management for inpatients.
• Support patients transitioning to appropriate levels of care.
• Collaborate with healthcare team and patients to optimize LOS and service use.
• Develop and implement a comprehensive care plan addressing physical, mental, and psychosocial needs.
• Operate in hospitals-only setting as part of the inpatient care team.

Job description

Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/ Registered Nurse (RN)!

This position is eligible for up to a $20,000 sign-on bonus for qualified candidates.

Status: Full-Time,permanent position (40 hours)

Standard working hours: 8:00 AM - 4:30 PM EST, Monday-Friday, with rotating weekends (Saturday or Sunday). A weekday off is provided when working a weekend day so hours do not exceed 40 per week.

Location: This is an onsite position at Sentara Norfolk General Hospital, 600 Gresham Drive, Norfolk, VA 23507.

Job responsibilities:

• The Inpatient Case Manager RN is responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS.

• Directly support those in hospital patients identified in need of comprehensive case management.

• Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions.

• Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver.

• Functions in one of the following practice settings: Hospitals Only

Education:

• Bachelor's Degree in Nursing preferred

Certification/Licensure

• Registered Nurse (RN) in VA or NC or Compact/Multi-state REQUIRED

• BLS within 90 days of hire.

• Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.

Experience:

• 1 year of nursing experience REQUIRED

• 1 year acute care case management preferred
