Job summary

Sentara Princess Anne Hospital in Virginia Beach is seeking an Inpatient Case Manager RN for a full-time, 40-hour position.

You will coordinate care across the continuum, support hospital patients, and develop comprehensive care plans with focus on transitions and clinical appropriateness.

Qualifications

• Bachelor's Degree in Nursing preferred.
• Registered Nurse (RN) license in VA/NC or Compact/Multi-state required.
• BLS certification within 90 days of hire.
• Case Management certification within one year of eligibility.
• Minimum 1 year of nursing experience.

Responsibilities

• Coordinate and facilitate comprehensive care management for patients across the healthcare continuum.
• Support hospitalized patients identified for case management needs.
• Promote effective utilization of health services and monitor transitions with the care team.
• Develop and implement a comprehensive care plan addressing physical, mental, and psychosocial needs.
