Job summary

Care Lync is seeking a dedicated Support Coordinator/Service Coordinator to manage individualized care plans for adults with intellectual, developmental, and physical disabilities. You will coordinate services, monitor outcomes, and support participants in accessing housing, medical and community services.

Responsibilities include assessing needs, developing ISP-based plans, ensuring provider choices, and maintaining confidentiality.

Qualifications

BS/BA degree in related course work.
Minimum of 3 years related experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities.
Experience with Microsoft Office, state-maintained databases.
Maintain CPR/First Aid certification and updated state related Clearances.
Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration.
Inform supervisor immediately of changes in auto insurance or license status.

Responsibilities

Complete assessments to identify participant needs and desires for service delivery.
Develop a Person-Centered Service Plan for each participant and update annually or with changes.
Ensure participant choice of providers by giving information for informed decisions.
Coordinate with other care coordinators to ensure seamless service delivery.
Research and develop alternative solutions to participant needs.
Maintain confidentiality of sensitive information and participate in trainings.
