Job summary

Care Lync is seeking a dedicated Support Coordinator to manage caseloads for adults with IDD, autism, and physical disabilities. The role involves building individualized care plans, coordinating services, and monitoring outcomes to ensure cost-effective supports that promote independent living.

The position emphasizes participant-driven planning, collaboration with providers, and ensuring access to housing, medical, social, vocational, and community services.

Qualifications

• Minimum 3 years experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities.
• MS Office proficiency and experience with state-maintained databases.
• Maintain CPR/First Aid certification and state-related clearances.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan and update annually or with significant change.
• Ensure participant choice of providers by giving information for informed decisions.
• Ensure Behavioral Support Plan and Crisis Plan align with ISP.
• Report incidents of abuse, neglect, or exploitation to supervisor.
• Coordinate with other care coordinators to ensure seamless service delivery.
• Research and develop alternative solutions to participants’ needs.
• Maintain confidentiality of sensitive information.
• Attend trainings and recertification classes.
• Consult with supervisors to consider participant needs and limitations.
• Travel up to 50% to participants as needed.
