Job summary

Care Lync seeks a dedicated Support Coordinator/Case Manager to empower adults with disabilities and aging individuals to live independently. You will assess needs, develop ISP plans, and coordinate with providers while advocating for client choices.

Qualifications

• Bachelor's degree in social work, psychology, human services, or related field.
• Strong organizational, communication, and advocacy skills.
• Willingness to travel up to 50% to meet with participants in homes and communities.
• Valid driver's license, auto insurance, and reliable transportation.
• Commitment to maintaining required certifications and state clearances.

Responsibilities

• Conduct person-centered assessments to identify participants' needs, preferences, and goals.
• Develop, implement, and update Individual Support Plans (ISP) to ensure appropriate services.
• Provide participants with choice and advocacy in provider and care options.
• Coordinate with medical professionals, family, and community resources to deliver high-quality care.
• Monitor and evaluate services for safety, satisfaction, and progress toward independence.
• Maintain documentation, confidentiality, and compliance with program standards.
• Participate in ongoing training and professional development.
