Job summary

Care Lync is seeking a dedicated Support Coordinator to help adults with disabilities and aging adults access housing, medical care, and community resources. You’ll develop and update Individual Support Plans, advocate for participant choices, and coordinate with a network of professionals to ensure high-quality, person-centered care.

Qualifications

Bachelor's degree required in social work, psychology, human services, or related field.
Minimum 3+ years of case management or support coordination experience.
Ability to travel up to 50% to meet participants in homes and communities.

Responsibilities

Conduct person-centered assessments to identify needs, preferences, and goals.
Develop, implement, and update Individual Support Plans (ISP).
Provide choice and advocacy to help participants access services.
Coordinate with medical professionals, behavioral specialists, family, and community resources.
Monitor services for safety, satisfaction, and progress toward independence.
Maintain documentation and ensure confidentiality and compliance.
