Care Lync is seeking a dedicated Support Coordinator to help adults with disabilities and aging individuals access housing, medical care, and community resources. You will conduct person-centered assessments and develop Individual Support Plans (ISP) to ensure services align with each participant's goals.

Position requires independent work and up to 50% travel to participants' homes and communities. You will maintain confidential documentation, collaborate with medical professionals and family,

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Comfort working independently and traveling up to 50% to meet with participants in their homes and communities.
• Valid driver's license, auto insurance, and reliable transportation.
• Commitment to maintaining required certifications and state clearances.

Responsabilités

• Conduct person-centered assessments to identify needs, preferences, and life goals.
• Develop, implement, and regularly update ISP to ensure participants receive the right services.
• Provide participants with choice and advocacy, helping them make informed decisions about providers and care options.
• Coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver seamless, high-quality care.
• Monitor and evaluate services to ensure participants' safety, satisfaction, and progress toward independence.
• Support both individuals with disabilities and members of the aging population in accessing the community resources they need.
• Maintain documentation, confidentiality, and compliance with all program and ethical standards.
• Participate in ongoing training and professional development to strengthen your skills and expand your impact.
