Job summary

Care Lync is seeking a Support Coordinator to help adults with disabilities and seniors access housing, medical care, and community resources. You will conduct person-centered assessments, develop ISP plans, and advocate for participants to ensure services meet their needs.

The role involves coordinating with healthcare professionals and families, traveling to participants' homes, and maintaining compliant documentation. Care Lync offers training and clear career growth paths in social services.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Comfort working independently and traveling up to 50% to meet with participants in their homes and communities.
• Valid driver's license, auto insurance, and reliable transportation.
• Commitment to maintaining required certifications and state clearances.

Responsibilities

• Conduct person-centered assessments to identify participant needs, preferences, and goals.
• Develop, implement, and update Individual Support Plans (ISP).
• Provide participants with choice and advocacy in care options.
• Coordinate with medical and behavioral professionals, family, and community resources.
• Monitor and evaluate services to ensure safety, satisfaction, and independence progress.
• Assist both individuals with disabilities and aging population in accessing community resources.
• Maintain documentation, confidentiality, and compliance with program standards.
• Participate in ongoing training and professional development.
