Dixon Social Interactive Services is seeking a compassionate Care Manager Extender to support Tailored Care Management in helping members navigate healthcare, behavioral health, and social service systems.

You will work under supervision, assisting with engagement, care coordination, and resource navigation while ensuring compliance with Tailored Care Management functions. This is an in-person role based in Wilson, NC.

Qualifications

• Must be 18 years of age.
• High school diploma or equivalent (GED or certificate).
• Lived experience with I/DD/TBI, or behavioral health conditions per qualifying pathways.
• Two years of paid experience in related roles.

Responsabilités

• Assist with member engagement and outreach for accessing services.
• Support care plan understanding, referrals, and available services.
• Document member interactions accurately in the care management system.
• Follow up on referrals, appointments, and community resources.
