Qualifications

• Bachelor's degree in Human Services, Social Work, Public Health, Marketing, Communications or related field preferred.
• Equivalent experience may be considered.

Responsibilities

• Lead community engagement, partnerships, and referral development.
• Represent the organization at health fairs and community events.
• Coordinate outreach plans and track engagement metrics.
• Support Tailored Care Management activities and administrative tasks.
• Maintain updated resource directory and client navigation.
• Prepare reports on outreach and referral trends.
