Job summary

Dixon Social Interactive Services, Inc. seeks a Care Manager Extender to assist in delivering integrated, whole-person care for Tailored Plan members in North Carolina.

You will work under a Care Manager to perform outreach, coordinate services, and support transitions within the care team. The role focuses on behavioral health, physical health, I/DD, TBI, LTSS, and community resources, ensuring smooth referrals, ongoing engagement, and accurate documentation per DSIS policies and Tailored Care

Qualifications

• Be at least 18 years old.
• High school diploma or equivalent.
• Complete Tailored Care Management training.
• Be supervised by a Care Manager and meet one of these pathways: lived experience with I/DD or TBI; Certified Peer Support Specialist with behavioral health; parent/guardian of an I/DD/TBI or behavioral health element; or two years paid extender experience.

Responsibilities

• Perform outreach, engagement, and follow-up with members.
• Coordinate services, appointments, and transportation; provide reminders.
• Support case conferences and documentation; track referrals and resources.
• Promote wellness, community integration, and self-sufficiency within scope of the role.
