Job summary

ncgCARE in Wilson, NC is hiring a Care Management Extender to support the Tailored Care Management program. The role requires managing high volumes of member calls, coordinating care, and linking members to housing, food resources, and community programs.

The ideal candidate has lived experience with a behavioral health condition, a Certified Peer Support Specialist credential, and at least two years of related paid experience working with Tailored Care Management eligible populations.

Qualifications

• High school diploma or equivalent and at least 18 years old.
• Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist.
• Two years of paid experience performing Extender Functions with Tailored Care Management eligible population.

Responsibilities

• Conduct regular outreach to members by phone and in person to assess needs and monitor progress.
• Connect members and families to healthcare services, housing assistance, food resources, and community programs.
• Assist in developing and updating individualized care plans to support health and wellness goals.
• Coordinate communication among healthcare providers, members, and families to ensure continuity of care.
