Job summary

Epiphany Family Services is seeking a TCM Care Manager Extender to support tailored care management programs in Gaston County. The role blends in-office, field, and remote work with travel as needed. You will engage members, coordinate services, and help enroll clients into Tailored Care Management services.

The position emphasizes person-centered planning, Medicaid knowledge, and strong communication. Hybrid hours and a comprehensive benefits package are provided to fit client needs and program

Qualifications

• Must be a Certified Peer Support Specialist or equivalent.
• Experience with community health workers or navigators is valued.
• Knowledge of Medicaid benefits is a plus.

Responsibilities

• Perform outreach, engagement, and follow-up with members.
• Coordinate services and appointments; arrange transportation as needed.
• Conduct introductory engagement calls to enroll members into TCM services.
• Support care team with referrals and community resources.
