Job summary

Carolina Outreach in Wilson, NC is seeking a Care Management Extender to support Tailored Care Management for behavioral and medical care needs. The role involves coordinating with clinicians, agencies, and families to ensure holistic, person-centered care and timely follow-up.

The ideal candidate brings lived experience or relevant caregiving background, strong communication skills, and the ability to manage a high volume of calls and referrals in a performance-driven environment.

Qualifications

• A high school diploma or equivalent; Must be 18 years or older.
• Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist or a parent/guardian of a related individual, or have two years of paid experience in related functions.
• Upon hire, completion of onboarding training with Tailored Care Management requirements within 60 days.

Responsibilities

• Conduct regular outreach to members by phone and in person to assess needs, provide support, and monitor progress.
• Connect members and families to healthcare services, housing assistance, food resources and community programs.
• Assist in developing and updating individualized care plans to support health and wellness goals.
• Coordinate communication among healthcare providers, members and families to ensure continuity of care and follow-up on referrals.
