Qualifications

• Minimum of one year experience in supportive housing.
• Two years of experience in independent living services in a social service setting.

Responsibilities

• Conduct assessments and interviews with clients.
• Develop and update action plans, case notes, and documents.
• Provide ongoing life skills development.
• Connect clients to medical and supportive counseling services.
• Guide clients through application processes for services.
