Compass Career Solutions, sister company to Dungarvin, seeks a Supportive Housing Case Manager for in-office work and community outreach. The role includes assessments, life skills coaching, and referrals to health and social services. A $1,500 retention bonus is available for eligible hires.

Monday–Friday daytime schedule with some nights/weekends. The position emphasizes documentation, collaboration with teams, and transportation support for clients, with a focus on stability and independence

Formación

• Bachelor's degree in social services or related field with at least one year of experience in supportive housing coordination.
• High School Diploma or GED with at least two years of experience in supportive housing coordination or related social services.

Responsabilidades

• Conduct assessments and interviews with clients to determine eligibility.
• Develop and update action plans, case notes, and related documents.
• Provide ongoing life skills development to improve independence and quality of life.
• Document care and follow support plans with clear communication to internal and external teams.
• Provide targeted interventions when clients are at risk of eviction.
• Connect clients to medical, mental health, and other supportive services.
• Refer clients to community services and assist with SSI/SSDI/Medicaid applications.
• Occasionally visit clients' homes and coordinate with property management for wellness checks.
• Travel within the area to provide client transport as needed.
• Represent Compass at meetings and community events.
