Job summary

Compass Career Solutions, sister company to Dungarvin, seeks a Housing Case Manager to provide assessment, life skills development, and case management for homeless individuals and families in the Tacoma area. This in-office role involves community outreach and some travel.

You will conduct eligibility interviews, develop action plans, connect clients to medical and social services, and coordinate referrals and paperwork for SSI/SSDI, Medicaid, Medicare, and SNAP. Training provided.

Qualifications

• Bachelor's Degree in social services or related field with one year housing coordination experience.
• High School Diploma or GED with two years housing or independent living services coordination.
• Must be 21+ years old and have a valid driver's license.

Responsibilities

• Conduct assessments and interviews to determine client eligibility.
• Develop action plans, case notes, and related documents.
• Provide ongoing life skills development to improve independence and quality of life.
• Document care, follow support plans, and collaborate with internal/external staff.
• Coordinate referrals to medical, mental health, and counseling services.
• Travel throughout the area to assist clients and perform wellness checks.
