Qualifications

• Bachelor's Degree in social services or related field and minimum one year experience in coordinating supportive housing or independent living services.
• High School Diploma or GED and two years of relevant experience in coordinating supportive housing or independent living services.

Responsibilities

• Conduct assessments and interviews to determine eligibility.
• Develop and update action plans, case notes, and client files.
• Provide ongoing life skills development to improve independence and quality of life.
• Document care and follow support plans with clear communication.
• Connect clients to medical, mental health, and counseling services.
• Refer clients to community services and supervise referrals.
• Assist with SSI/SSDI/Medicaid/Medicare/SNAP applications and re-determinations.
• Occasionally visit clients' homes and coordinate with property management for wellness checks.
• Maintain outcomes data and report progress as directed.
• Attend internal and external meetings and provide transportation as needed.
