Job summary

Compass Career Solutions, sister company to Dungarvin, is seeking a housing case manager to deliver client-focused support and life skills development in a non-remote, in-office role within the community outreach framework.

You will conduct assessments, develop action plans, document care, and connect clients to housing and supportive services, including SSI/SSDI and Medicaid where applicable. Strong communication and organizational skills are essential.

Qualifications

• Bachelor's Degree or two years of relevant housing experience required.
• Experience coordinating supportive housing or independent living services preferred.
• Must be able to read, write, and document effectively.

Responsibilities

• Conduct assessments and interviews to determine eligibility.
• Develop action plans, case notes, and client files.
• Provide life skills development and support to improve independence.
• Coordinate referrals to medical, mental health, and community services.
• Travel within the area to assist clients and participate in meetings.
