Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Saint Paul, MN. The role is PRN and focused on delivering high-quality rehab services to residents within a compliant, interdisciplinary team.

You will perform assessments, develop treatment plans, and document care in NetHealth EMR, while adhering to CMS requirements and company procedures. State licensure is required and ongoing education encouraged.

Qualifications

• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
• Clinical competence in evaluation, plan development, and therapy.
• Compliance with CMS documentation expectations.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Document and bill therapy services in accordance with procedures.
• Develop short and long-term treatment goals for residents.
• Evaluate residents to inform treatment plans and interventions.
• Communicate evaluation findings and plans to residents and care teams.
• Complete compliant documentation in NetHealth EMR.
