Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Shakopee, MN. This PRN position supports therapy services under CMS guidelines and participates in interdisciplinary care planning to ensure high-quality resident outcomes.

You will evaluate residents, develop treatment plans, and document progress while maintaining compliance and professional standards. Ongoing education and licensure are expected.

Qualifications

• Licensed physical therapist with active license in state of practice.
• Commitment to ongoing continuing education and professional development.
• Bachelor's degree in Physical Therapy from an accredited program.

Responsabilités

• Provide physical therapy services to residents following standards and productivity guidelines.
• Document and bill therapy services per procedures.
• Evaluate residents and develop treatment plans with short and long-term goals.
• Communicate findings and recommendations to residents, families, and interdisciplinary team.
• Screen residents to determine need for intervention and plan treatment accordingly.
• Complete CMS-compliant documentation in NetHealth EMR.
• Perform other duties to support department objectives.
