Job summary

Evolve Therapy Services, LLC seeks a licensed Physical Therapist to provide in-house SNF services in Saint Paul, MN. The role supports planning, evaluating, and directing physical therapy for residents, with emphasis on quality care and compliant documentation.

The position is PRN, with opportunities for growth within the organization. Active licensure in Minnesota and a BS in Physical Therapy are required; ongoing education is encouraged.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct regular screening of residents to determine need for intervention/treatment.
• Screen residents referred to physical therapy to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Conduct specialized evaluations as indicated.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings, treatment plan, and recommendations to residents, families, and interdisciplinary team members.
• Complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems.
