Qualifications

• An active Physical Therapy license in the state(s) of practice.

Responsibilities

• Provide physical therapy services to residents per standards and productivity guidelines.
• Document and bill PT services following procedures.
• Evaluate residents to plan treatment and set goals.
• Develop and implement treatment plans with short/long-term goals.
• Communicate findings to residents, families, and inter-disciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
